Transition Program

Rich Township High School District 227 Transition Program serves qualifying young adults with disabilities ages 18 through the day before their 22nd birthday.  The primary focus of the program is to support students to transition from high school to adult life through vocational, community, and independent living instruction that builds on a student’s interests and abilities.  

The transition program is available for students who have completed graduation requirements and need transitional supports in order to achieve the highest level of independence possible in their adult life. Participation in the program is determined by the IEP team. Each student has his/her own plan and schedule designed to best meet their individual needs based on their unique Transition Plan.  Instruction is provided by highly qualified special education teachers in collaboration with social workers, speech therapists, occupational therapists, physical therapists, paraprofessionals, guidance counselors, and job coaches.

  • Social & Community Living Skills and Participation

  • Academic Support

  • Vocational Training

  • Leisure Exploration

  • Self-Advocacy 

Upon graduation all students will be given a Summary of Performance. Young adults will leave the program with a clear plan for postsecondary education, vocational training, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation based on their individual abilities, interests, and needs. These plans will link students with postsecondary institutions or gainful employment. They will also address community access and living arrangement when appropriate.
