Illinois School Code requires all school-age children to attend school. The purpose of Homebound Instruction is to provide eligible students access to continuous instruction at home if they are unable to attend school due to a temporary medical, psychiatric, or behavioral condition estimated to be at least two (2) consecutive weeks to six (6) months in duration.  This may include ongoing intermittent absences.  Providing this instruction is a team effort that involves parents/guardians, health providers, guidance counselors, classroom teachers, homebound teachers, administrative staff, and the student.   

Homebound service requires completion of ISBE Form 34-58 Medical Certification for Home/Hospital Instruction from a licensed Medical Doctor, Doctor of Osteopathic Medicine, Advanced Practice Registered Nurse, or Physician Assistant. Statements from any medical personnel other than a licensed M.D., D.O., APRN, or PA, (e.g. psychologists, physical therapists, or chiropractors), are not accepted by the state. The form is given to the student’s guidance counselor to begin the approval process. 

This form must include: 

• Diagnosis and impact on ability to participate in education

• Begin Date

• Anticipated Return Date
