How to fill out a technology request form

A Technology Request eForm (below) must be completed when requesting any of the following services from the Information Technology Department.

• Changes to your network or e-mail account.
• Any staff ID/Badging building access changes.
• Request for wireless network access for your personal wireless device.

If the request is for additional building access using your ID, your building principal must approve your request prior to you submitting this form.

The Technology Request eForm can be completed on-line and forwarded to Pam Jonas in the Information Technology Department.
